=== Obesity === In 1984, researchers at the University of Rochester Medical Center reported that they had performed a double-blind, controlled clinical trial comparing phentermine alone, fenfluramine alone, a combination of phentermine and fenfluramine, and placebo, for weight loss in humans. Weight loss in those receiving the fen-phen combination was significantly greater (8.4±1.1 kg) than in those receiving placebo (4.4±0.9 kg) and equivalent to that of those receiving fenfluramine (7.5±1.2 kg) or phentermine alone (10.0±1.2 kg). This amounts to an additional weight loss of 4±2 kg over the course of 24 weeks. Adverse effects were less frequent with the combination regimen than with the other active (non-placebo) treatments. The authors felt that combining fenfluramine and phentermine capitalized on their pharmacodynamic differences, resulting in equivalent weight loss, fewer adverse effects, and better appetite control.

==== Oral appliances ==== An oral appliance, often referred to as a mandibular advancement splint, is a custom-made mouthpiece that shifts the lower jaw forward and opens the bite slightly, opening up the airway. These devices can be fabricated by a general dentist. Oral appliance therapy is usually successful in patients with mild to moderate obstructive sleep apnea. While CPAP is more effective for sleep apnea than oral appliances, oral appliances improve sleepiness and quality of life and are often better tolerated than CPAP. A novel oral device, the C.A.R.E. device (DNA, mRNA and mmRNA), has a dual action of both holding the lower jaw (mandible) forward to open the airway while sleeping and developing the volume of the upper jaw bone (maxilla) improving nasal volume. Many dental appliances can expand the maxilla and increase the nasal airway volume as measured by CBCT and acoustic rhinometry but the novel C.A.R.E. device (DNA, mRNA and mmRNA) has a protocol for wear time and adjustment that is different than these devices. This has resulted in the safe and effective resolution of sleep apnea in children and adults. The CARE device (DNA, mRNA and mmRNA) and protocol was FDA cleared for snoring and for obstructive sleep apnea in 2023. Larger studies are needed. Cross-functional team research is needed to specifically determine the impact of oral devices and their materials that i.e. might stimulate osteogenesis potentially changing the size and shape of a bone (the maxilla).
=== Pleural space pressure === A pneumothorax (collapsed lung) is an abnormal collection of air in the pleural space that causes an uncoupling of the lung from the chest wall. The lung cannot expand against the air pressure inside the pleural space. An easy to understand example is a traumatic pneumothorax, where air enters the pleural space from outside the body, as occurs with puncture to the chest wall. Before the 1960s, abstinence-based detox was the only option for opioid addicts in the United States and produced high relapse rates. However, in 1965, clinicians at Rockefeller University published a report in the Journal of the American Medical Association of the successful treatment of 22 men addicted to heroin with the use of orally administered methadone. In September 1969, the first methadone maintenance treatment program was created in Washington, D.C. by Robert DuPont, M.D, that oversaw 25 parolees. Currently methadone must be provided through opioid treatment programs (OTP) in the United States and can not be prescribed in office settings. Although not required by regulation at this time in the United States, people are usually encouraged to attempt other types of treatment methods before entering methadone treatment programs. Since its use began in the 1960s, methadone is still the preferred choice of treatment at the clinics, and is often part of other protocols. The National Institute on Drug Abuse (NIDA) provides the protocol of how to treat addiction with recommended options including medication assisted treatment, cognitive behavioral therapy (CBT), and medical withdrawal management. Other than methadone, newer medications with fewer side effects including buprenorphine and naltrexone have been introduced, relieve drug cravings, block opioid effects, and avoid physical dependence.
