=== Self-administration === As opposed to intramuscular or intravenous injections, subcutaneous injections can be easily performed by people with minor skill and training required. The injection sites for self-injection of medication are the same as for injection by a healthcare professional, and the skill can be taught to patients using pictures, videos, or models of the subcutaneous tissue for practice. People who are to self-inject medicine subcutaneously should be trained how to evaluate and rotate the injection site if complications or contraindications arise. Self-administration by subcutaneous injection generally does not require disinfection of the skin outside of a hospital setting as the risk of infection is extremely low, but instead it is recommended to ensure that the site and person's hands are simply clean prior to administration.
The Albert Einstein College of Medicine is a private medical school in New York City. Founded in 1953, Einstein is an independent degree-granting institution within the Montefiore Einstein Health System. Einstein hosts MD, PhD, and master's programs. === Pharmacodynamics === Flmodafinil is a selective dopamine reuptake inhibitor (DRI). Its affinity (Ki) for the DAT is 4,090 nM. At the serotonin transporter (SERT), its affinity (Ki) was 48,700 nM (12-fold lower than for the DAT), and it had negligible affinity for the sigma σ1 receptor (Ki > 100,000 nM). The drug has been found to block the dopamine transporter (DAT) by 83%, to a greater extent than methylphenidate without unfavorable concomitant adrenergic effects. The drug is an atypical DRI similarly to modafinil. The affinities for the DAT of flmodafinil's enantiomers and modafinil have also been studied. The affinities (Ki) were 5,480 nM for armodafinil ((R)-modafinil), 2,970 nM for (S)-(+)-flmodafinil (JBG1-048), and 4,830 nM for (R)-(–)-flmodafinil (JBG1-049). Their affinities for the SERT and σ1 receptor have also been reported. Similarly to modafinil, (S)-(+)-flmodafinil and (R)-(–)-flmodafinil increase dopamine levels in the nucleus accumbens in animals. They have been found to increase dopamine levels by up to 150 to 200% of baseline at the highest assessed dose. These increases are much smaller than those elicited by amphetamine or cocaine. In a study comparing the wake-promoting effects of flmodafinil and modafinil, flmodafinil was found to maintain wakefulness over a significantly longer timeframe than modafinil. While the administration of neither compound resulted in sleep rebound, flmodafinil perturbed sleep architecture to a lesser degree than modafinil.
=== Pharmacodynamics === Enobosarm is a selective androgen receptor modulator (SARM), or a tissue-selective mixed agonist or partial agonist of the androgen receptor (AR). This receptor is the biological target of endogenous androgens like testosterone and dihydrotestosterone (DHT) and of synthetic anabolic steroids like nandrolone and oxandrolone. The affinity (Ki) of enobosarm for the AR is high and was measured as 3.8 nM in one study, or approximately 16.8% of that of DHT. Enobosarm shows enantioselectivity for the AR and has similar but somewhat lower potency than DHT in terms of activating the receptor. In addition to general activation of the AR, enobosarm induces the N/C interaction (the interaction of the amino terminus and carboxyl terminus) of the AR less potently than does DHT, but in any case promotes the N/C interaction concentration-dependently and to the same maximal extent as DHT. The AR is widely expressed in tissues throughout the body, including in the prostate gland, seminal vesicles, genitals, gonads, skin, hair follicles, muscle, bone, heart, adrenal cortex, liver, kidneys, and brain, among others.
